Home care in Tenafly

Bergen County runs ten senior activity centres. They are in Bergenfield, Palisades Park, Elmwood Park, Ridgefield Park, Garfield, East Rutherford, Midland Park, Hackensack, North Arlington and Wallington. Ridgewood, Allendale, Ho-Ho-Kus and part of Saddle River are all served from Midland Park, which is a different municipality from any of them.

Tenafly is not in that position. There is a senior centre on South Summit Street, in Tenafly, which means the walk or the short lift that gets somebody there is a walk or a short lift rather than a cross-county journey nobody can make.

That difference is larger than it sounds, and it is the single most useful thing to establish about any Bergen town before planning around public provision. A centre four miles away in another borough is a real service that a person who has stopped driving cannot use.

It also changes what families here should be buying. Where the centre is local and usable, the gap is rarely daytime company. It is the hours the centre is shut, which are the same hours everywhere: evenings, weekends, and the fortnight after something happens.

Discharges in Tenafly generally come back through Englewood Hospital, a few minutes down the hill on Engle Street. As with every town near its hospital, proximity is genuinely reassuring and does almost nothing about the hours.

The senior centre is closed at the weekend. County transport does not run on demand. The clinical team who know the case are back on Monday. A Friday afternoon discharge lands in a window where none of the things a family has been told to rely on are available, and the person who was steady in a hospital bed is not steady on their own stairs.

This is the arrangement we are asked for most often in eastern Bergen, and it is a short one. Cover across the first weekend and the following week, then a review, rather than an open-ended package agreed while everybody is frightened.

It is also the arrangement most likely to end after a fortnight, which is the correct outcome. Somebody who recovers well should stop buying care, and an agency that treats that as a lost customer has the incentive pointed the wrong way.

Tenafly's median home dates to 1958, and the borough sits on the slope up from the Hudson palisades. Both facts matter and the second is the one nobody mentions.

Post-war construction of that period is comparatively kind: wider stairs than a pre-war colonial, a reasonable chance of a bathroom on the entry floor, doorways that a walking frame fits through. Where these houses are difficult is the approach. Split-level entries, a run of steps from the pavement to the front door, and driveways that pitch.

A curb-to-curb transport service delivers somebody to the curb. On a sloping street with six steps up to the door, the curb is not where the journey ends, and the part that defeats people is the part no service covers.

So the first thing worth checking in a Tenafly house is the route from the pavement to the kitchen, not the route from the bedroom to the bathroom. It is the reverse of the pre-war townships an hour south, and a plan written on the assumption that stairs mean interior stairs will get the hours wrong.

Everything we provide is daily-living. Personal care, bathing, dressing, mobility support and transfers within a caregiver's training, medication reminders, meal preparation, the household routine, companionship, and getting somebody to an appointment and back on the days county transport does not run.

Nothing about the line beyond that is soft. No skilled nursing, no wound care, no injections, no therapy, no medication administration or dose adjustment, and no clinical assessment. Where a clinical plan already exists we work under it and we do not direct it.

In a borough with usable local provision, the honest answer is frequently that a household needs less than it arrived intending to buy. Weekends and the first fortnight after a hospital stay, rather than a standing weekday presence the senior centre already provides better and free.

And if the real question is whether somebody can stay in a house with six steps to the front door, that deserves a straight conversation rather than being deferred by adding hours until it goes away. It is the family's decision. We would rather it were an informed one.
