Home care in Allendale

Allendale is a borough of under seven thousand people, and better than one in five of them is over 65. That is the highest share of any town in our coverage, higher than Cranford, higher than North Salem, higher than anywhere on the Connecticut shore.

Bergen County runs ten senior activity centres. They are in Bergenfield, Palisades Park, Elmwood Park, Ridgefield Park, Garfield, East Rutherford, Midland Park, Hackensack, North Arlington and Wallington. Read that list against a map of the county and the pattern is not subtle: not one of them is in the northwest estate belt.

Allendale's provision is the Northwest Senior Activity Center on Center Street in Midland Park. It is a real centre doing real work and it is in a different municipality, which means using it requires getting there, which is the thing the people most likely to need it can no longer reliably do.

None of that is a scandal. Counties put centres where the density is, and the density is not here. It does mean that the honest description of public provision in Allendale is thinner than a county directory makes it look, and families deserve to hear that before they plan around it.

This is the pattern across northwest Bergen and it is worth naming plainly, because it runs against what people assume. Alpine, Franklin Lakes, Upper Saddle River, Ho-Ho-Kus, Saddle River and Allendale are among the wealthiest towns in the state, and they have the least municipal older-adult infrastructure of anywhere we work.

The assumption is that money substitutes for provision, and up to a point it does. What it does not substitute for is proximity. A large house on a long driveway a mile from anything is a wonderful place to raise children and a genuinely difficult place to be eighty-five and no longer driving.

The practical consequence is that the first thing to go in a town like this is not care, it is contact. Somebody stops driving and their week quietly empties, and because the house is comfortable and the family is not worried about money, it can take a long time for anybody to notice.

So the arrangements we run in Allendale skew differently from the ones we run in a dense town. Fewer intensive personal-care hours, more standing blocks that put someone in the house on a rhythm and get the person out of it. That is a smaller purchase than families expect and it is usually the right first one.

Allendale's median home dates to 1965, which is late enough to matter. Post-war construction of that period tends to mean wider stairs than a pre-war colonial, more likelihood of a bathroom on the entry floor, and rooms that a walking frame can actually get through.

That is a real advantage and it is worth using rather than ignoring. A household here has more scope to adapt the building than a household in a 1938 township, and where an adaptation solves the problem it is worth pricing against hours before committing to either. We are not the right people to tell you which comes out cheaper in your house, and we would rather you asked an occupational therapist than took our word for it.

Where the newer stock does not help is scale. These are large houses, and a large house is a great deal of walking for somebody who has become unsteady, quite apart from any stairs. The distance from the bedroom to the kitchen is a factor in a way it simply is not in a smaller home.

The way to tell which of those two things is governing a household is to look at the actual route somebody takes through their day. It is a short conversation in the house and a guess over the phone, and the guess is usually wrong in the expensive direction.

Everything we provide here is daily-living. Personal care, bathing, dressing, mobility support and transfers within a caregiver's training, medication reminders, meal preparation, the household routine, companionship, and getting somebody to an appointment and back, which in this borough is a large part of the value.

Nothing about the line beyond that is soft. No skilled nursing, no wound care, no injections, no therapy, no medication administration or dose adjustment, and no clinical assessment or advice. Where a clinical plan already exists we work alongside it and we do not direct it.

We will also say when the answer is not us. If somebody in Allendale is isolated rather than unsafe, a driving arrangement and a standing weekly commitment may do more for them than personal care will, and it costs a fraction as much. We would rather tell you that than sell hours against it.

And if the real problem is that a very large house has become unmanageable, that is worth saying out loud too. It is a genuine trade-off, it is the family's decision rather than ours, and it deserves discussing properly rather than being deferred by adding hours until the question goes away.
