Companion Care and Home Care Agencies in New York
New York licenses some home care agencies and not others, and the difference is worth knowing before you call anyone. Here is where the line falls, how to check a licensed agency, and what Endurance does and doesn't provide in New York.
Does a home care agency need a license in New York?
It depends on the care. In New York, an agency that provides nursing, home health aide or personal care services must hold a license from the State Department of Health. Endurance holds no New York home care license and provides companion care only, including live-in and 24/7 companion care.
The license
New York's licensed home care services agencies
New York calls a licensed agency a licensed home care services agency, or LHCSA. Under Public Health Law section 3605, no home care services agency that is engaged in providing, directly or through contract, nursing services, home health aide services or personal care services may operate without a license from the commissioner of health.
The law defines personal care services as help with personal hygiene, dressing, feeding and household tasks essential to the patient's health, prescribed by a physician under a plan of home care supervised by a registered professional nurse. That is why a licensed agency is the route for that kind of care in New York. The Department of Health monitors the care LHCSAs provide, and these agencies offer home care to clients who pay privately or have private insurance.
A different kind of provider
What a companion-only provider is
New York's own business portal says that an agency that provides solely companionship services with light housekeeping, with no nursing, home health aide or personal care services, does not need the LHCSA license.
Endurance Home Care holds no New York home care license and provides companion care only. Companion care means company, meals, light housekeeping, errands, rides and reminders. Caregivers give reminders for medications and do not give hands-on help. That includes live-in companion care and 24/7 companion care, where caregivers work awake shifts through the day and night.
For licensed agencies
Training hours for licensed agency aides in New York
These training rules apply to aides who work for licensed home care agencies. They do not describe Endurance's companion caregivers.
Home health aide (HHA): 75 hours
The Department of Health requires an HHA trainee to receive a minimum of 75 hours of training, including at least 59 hours of classroom and 16 hours of supervised practical training. The agency that employs the aide must provide nursing supervision and 12 hours of in-service training each year.
Personal care aide (PCA): at least 40 hours
A personal care aide, at Level II, holds a certificate from a Department of Health or Education Department approved program that must offer a minimum of 40 hours of training. A PCA is permitted to perform fewer health-related tasks than a home health aide, and an upgrade to HHA takes 35 more hours.
A quick lookup
How to check a licensed agency on the Department of Health site
The Department of Health lists licensed home care services agencies on its home care profiles site. Search for the agency's name, and compare the name and the license to what the agency told you. A licensed agency must also be registered with the Department of Health under Public Health Law section 3605-b to operate or receive payment for its services.
One thing to know: a company that provides companion care only may not appear on that search, because that scope does not require the license. That is expected. If a company says it provides nursing, home health aide or personal care services and you can't find it there, ask why before you go any further.
Honest guidance
When your family needs a licensed agency instead of companion care
Companion care is the right fit for a lot of families: someone for company, meals, errands and rides, or a caregiver overnight so you can sleep. It isn't the right fit for every need, and we'd rather you know that before you choose.
Endurance does not provide nursing, home health aide services or personal care in New York. If your parent needs them, look for an appropriately licensed provider: in New York, an agency that provides those services must be licensed by the Department of Health, and some nurses also practice on their own under their professional license. If your parent is recovering from a hospital stay and qualifies, Medicare covers home health only when you need part-time or intermittent skilled services and are homebound, and a Medicare-certified home health agency must provide it.
Many families need both kinds of help at different times of day. You can arrange a licensed agency for the care that needs one and companion care for the rest of the day and night. Our care team will talk through which is which, and will say so when a licensed agency is what you need.

Questions families ask
Does a companion care company need a license in New York?
New York's business portal says an agency that provides solely companionship with light housekeeping, and no nursing, home health aide or personal care services, does not need the home care agency license. Endurance holds no New York home care license and provides companion care only. Ask any provider exactly what its caregivers do.
What does a New York home care license cover?
Public Health Law section 3605 requires a Department of Health license for an agency that provides, directly or by contract, nursing services, home health aide services or personal care services. Licensed agencies serve clients who pay privately or have private insurance, and the Department of Health monitors their care.
How do I check a home care agency in New York?
Use the Department of Health's home care profiles site and search for licensed home care services agencies. Compare the name and license to what the agency gave you. A company that provides companion care only may not appear there, because that scope does not require the license, which is expected for that kind of provider.
What training do home health aides have in New York?
At licensed agencies, a home health aide completes at least 75 hours of training, including 59 classroom hours and 16 supervised practical hours. The employing agency provides nursing supervision and 12 hours of in-service training each year. Personal care aides complete a program of at least 40 hours. Endurance's New York caregivers provide companion care only.
When do I need a licensed agency instead of companion care in New York?
When your parent needs nursing, such as wound care or injections, or when a doctor has ordered personal care or home health aide services. Those call for an appropriately licensed provider: in New York an agency that provides them needs a Department of Health license, and a nurse needs a professional license. If it's company, meals, errands, rides or someone overnight, companion care can fit. Our care team will say which you need.
Does Endurance hold a New York home care license?

Care in New York is arranged from our Stamford, Connecticut office, and Endurance is registered in Connecticut (HCA.0002450) and New Jersey (HP0435100).
Where does Endurance provide companion care in New York?
In Westchester County and Manhattan, Endurance arranges companion care by the hour, overnight or around the clock. Live-in companion care is available in the rest of New York. Care is arranged from our Stamford office at 60 Long Ridge Rd, Suite 214, and a registered nurse visits first to write the care plan with your family.
Does Medicare pay for companion care in New York?
No. Medicare covers home health only for part-time or intermittent skilled care when you are homebound, and a Medicare-certified agency must provide it. It does not pay for 24-hour care at home. Companion care is private pay with Endurance, which does not accept insurance directly but helps families with long-term care policy paperwork.
Can I use a licensed agency and companion care together?
Yes. Some families arrange a licensed agency for the care that needs a license and companion care for company, meals, errands and the overnight hours. Ask each provider to put in writing what it will and will not do, so nothing falls between them. Our care team can help you sort out who does what.
