What is a personal care aide?

A personal care aide (PCA) helps a person at home with personal hygiene, dressing and eating, along with household tasks. New York requires a certificate from an approved program of at least 40 hours. New Jersey says personal care is performed by licensed or certified personnel. In Connecticut, homemaker services include personal hygiene under the statute.

The short answer

What is personal care?

Personal care is hands-on help with the routines of the body: bathing, dressing, grooming, toileting, eating and moving safely. It's what most people mean when they say their mother "needs help getting ready" each morning. At Endurance, personal care includes everything in companion care, plus that hands-on help.

State law uses different words for it, and that matters when you compare agencies. The definitions below come from each state's own statutes.

What certificate does a New York PCA need?

New York's Public Health Law defines personal care services as help with personal hygiene, dressing, feeding and household tasks essential to the patient's health, prescribed by a physician under a plan of home care supervised by a registered nurse. An agency that provides personal care services needs a license from the Department of Health.

The Department of Health says a personal care aide (Level II) holds a certificate from an approved NYSDOH or NYSED training program that must offer a minimum of 40 hours, and performs fewer health-related tasks than a home health aide. Programs can upgrade a personal care aide to a home health aide with 35 more hours.

Endurance doesn't hold a New York home care license, so in New York it provides companion care only, and hands-on personal care is not part of it. Companion care there means company, meals, light housekeeping, errands, rides to appointments and reminders.

How does New Jersey handle personal care?

New Jersey's statute defines personal care services as services performed by licensed or certified personnel to assist with activities of daily living that may involve physical contact: bathing, toileting, transferring, dressing, grooming, and help with walking or exercise. Companion services, by contrast, are basic supervision and socialization that do not include help with activities of daily living.

In practice that means you should ask what certification the caregiver holds, and who employs them. A firm that employs, places or refers people to provide companion, health care or personal care services in the home of a person with a disability or age 60 or older must register with the Division of Consumer Affairs. Endurance is registered in New Jersey as a Health Care Service Firm, HP0435100.

How does Connecticut handle personal care?

Connecticut doesn't use the title "personal care aide" in its homemaker-companion law. The statute defines homemaker services as supportive services, including cooking, household cleaning, laundry, personal hygiene and other household chores. The Department of Consumer Protection lists personal hygiene among the services a registered agency may provide, and rides to appointments too.

We found no state training-hour requirement for personal care aides in Connecticut. The law does require a full background check before hire, a written contract or service plan within seven days of starting service, and training for staff as mandated reporters. Medical services are excluded: DCP says registered agencies may not administer medications or shots, or provide nursing services such as assessing blood pressure or wound care.

Endurance is registered in Connecticut as a Homemaker-Companion Agency, HCA.0002450. Skilled nursing, home health aides and physical therapy come from Endurance Home Health, a separate company.

How is a PCA different from a home health aide or a CNA?

The three jobs overlap in the daily tasks. They differ in training, supervision and what each is permitted to do.

  • Personal care aide

Helps with hygiene, dressing, eating and the household around them. New York sets a 40-hour minimum for the certificate.

  • Home health aide

Does the same kind of hands-on work and, under a registered nurse's supervision, health-related tasks the nurse delegates. At least 75 hours in New York and under federal rules; 76 in New Jersey.

  • Certified nursing assistant

Completed a state nurse aide program of at least 75 hours federally, 100 in Connecticut, 90 in New Jersey. Most Endurance caregivers are CNAs.

When does a family need a personal care aide?

When the daily routine has become hard to do alone: getting out of bed, washing, dressing, using the bathroom, eating. If it's company, meals and errands that have slipped, companion care may be enough. If a nurse is managing a condition and delegating tasks, you may need a home health aide.

Medicare doesn't pay for custodial or personal care when it's the only care you need, so most families pay privately for it. A long-term care insurance policy may reimburse it, depending on the policy.

Questions families ask

What does PCA stand for?

PCA stands for personal care aide. It is an aide who helps with personal hygiene, dressing, feeding and household tasks. The title and the training behind it differ by state, so ask any agency what training the caregiver actually completed and who employs them.

What is the difference between a PCA and an HHA?

A home health aide completes more training and works under a registered nurse's supervision. In New York, a personal care aide holds a certificate from an approved program of at least 40 hours and performs fewer health-related tasks than a home health aide, whose program is at least 75 hours.

How many hours of training does a PCA need?

New York requires a certificate from an approved program that must offer a minimum of 40 hours. We found no state training-hour requirement for personal care aides in Connecticut. New Jersey says personal care is performed by licensed or certified personnel, so ask what the caregiver holds.

Is a personal care aide the same as a CNA?

No. A CNA completed a state nurse aide program, at least 75 hours federally and 100 hours in Connecticut. A personal care aide's training is set by the state's personal care rules. Ask what credential a caregiver holds. Most Endurance caregivers are CNAs.

What help can Endurance provide in New York?

In New York, Endurance provides companion care only: company, meals, light housekeeping, errands, rides and reminders. Companion care by the hour, overnight or around the clock is available in Westchester County and Manhattan, and live-in companion care is available in every county. Personal care with hands-on help is offered in Connecticut and New Jersey, where Endurance holds its registrations.

Can a Connecticut homemaker-companion agency help with bathing?

Connecticut's statute lists personal hygiene assistance within homemaker services, and the Department of Consumer Protection lists personal hygiene as permitted. Neither document spells out bathing, toileting or lifting, so ask the agency what its caregivers do, and what training they have for it. Endurance offers personal care in Connecticut.

Does insurance pay for a personal care aide?

Medicare doesn't pay for custodial or personal care when it's the only care you need. A long-term care insurance policy may reimburse personal care at home, up to a daily limit, depending on the policy. Endurance is private pay and helps families with the policy paperwork.
