Medicare Advantage Home Care Benefits in 2026

In 2026, KFF reports that in-home support services are offered as a supplemental benefit to only 10 percent of enrollees in individual Medicare Advantage plans and 38 percent of enrollees in Special Needs Plans.

Two plan types, two different odds

The benefit families are asking about is real, and it is uncommon. Medicare Advantage plans may offer in-home support services as a supplemental benefit, which is a different thing from a medical benefit every plan has to carry. Because it is optional, the honest answer to whether your plan covers home care is that it depends on which plan you hold. The 2026 figures below come from the same KFF analysis and they split by plan type, so start by finding your own row rather than the average of both.

Original Medicare does not sit underneath this and quietly fill the gap. It never has.

The home health benefit does not pay for 24-hour-a-day care at home, and it does not pay for custodial personal care when that is the only care a person needs. Those two exclusions are the baseline every plan is built on top of, which is why a supplemental benefit is the exception being asked about rather than the rule being confirmed.

How to check your own plan

Every plan publishes an evidence of coverage document and a schedule of supplemental benefits, and that list is the only thing that settles this. Read it for the exact phrase in-home support services, then call the number on the back of the card and ask the plan directly whether the benefit is included for your plan year, how many hours it allows, and which agencies it can be used with. Ask for the answer in writing. A plan representative saying a benefit exists and a schedule of benefits printing it are two different pieces of evidence, and only one of them survives a disagreement in November. Cross out the assumptions below before you start reading, because each one sends families to the wrong page of the booklet.

Whatever the booklet says

Plan on the supplemental benefit being a contribution rather than a plan. Endurance’s own answer to families is that most health insurance does not cover daily-living care, though long term care insurance may, and Medicare and most health insurance, including Medigap, do not pay for daily-living long term care at all. So the practical arrangement is usually a pairing: whatever hours a plan allows, carried by private pay or a policy for the rest of the week.

The policy version is worth reading before the plan version, because it is the one that tends to cover more hours. Start with long term care insurance and home care.
